Insurance Quote or Application Request Forms

ExcelLifeBenefits.com provides free life insurance quotes. To provide quotes, we must collect certain information including state of residence, date of birth, gender, height, weight, coverage amount desired, health classification of the proposed insured, and tobacco use. For simplified-issue and guaranteed-issue life insurance quotes, other health information may also be required.

Excel Life Benefits does not sell any submitted information. Excel Life Benefits may share information with life insurance carriers to provide the best service and products to you, our client to complete an insurance application.

All life insurance carriers have their proprietary rate guidelines based on the previously mentioned information. To provide quotes and rates, we must collect the data mentioned above.
